Chapter Enhancement Scholarships

Purpose: The purpose of the Chapter Enhancement Scholarship is to promote chapter educational activities and/or community programs that uphold the missions of SUNA and the SUNA Foundation within local SUNA chapters.

Description: Annually, four qualifying chapters will be eligible to receive a scholarship up to $200. All active affiliated SUNA chapters are eligible to apply for and receive the scholarship award. ​This is a privately funded scholarship and will be discontinued when available funds have been utilized.

Procedure: Chapters must submit a completed application by June 30. The SUNA Foundation Trustees and the Foundation Chair will review the applications. Chapters will be notified of their award status by August 1. The four chapters chosen will be announced during the Annual Conference.

Recipient’s Responsibility: Recipient chapters will be expected to submit documentation either written or electronically to the Foundation board regarding the educational offering or community program within 30-60 days from its conclusion. The documentation should include how the educational offering and/or community program benefited the chapter or greater population.

Criteria: All affiliated SUNA Chapters are eligible to apply for and receive the Chapter Enhancement Scholarship Award. Active chapter status will be verified with the SUNA Secretary. A current Chapter Review form should be on file. Chapters may apply annually as appropriate; previously awarded chapters will be ineligible to apply for the next three years. Chapters must submit their application complete with detailed description of the proposed educational and/or community programs by June 30.

Letter from Chapter Enhancement Scholarship Recipient

The Syracuse Chapter was awarded a Chapter Enhancement Scholarship in June 2018 to subsidize our participation in the "Spring into Health" fair in 2019. Our chapter attended the event, which was held on Sunday April 7, in conjunction with Natur-Tyme, a local health store, along with 30 other vendors. We used our scholarship to sponsor a booth. Chapter members volunteered at the booth and talked with participants about urologic health. We discussed four main topics: kidney stones, bladder health, hematuria, and prostate cancer. For each topic, we provided questions for participants to answer for a chance to win a free water bottle. More than 250 participants stopped at our booth and shared their experiences with us. SUNA fact sheets related to the main topics also were available at the table. On behalf of the chapter, I want to express our appreciation for having been awarded the scholarship. This community event was an extremely rewarding experience.

Chapter Start-Up Funds

Description: Start-up chapters may apply for a one time grant for $100 to support the growth and development of a new chapter in accordance with the missions of SUNA and the SUNA Foundation.

Eligibility Criteria

  1. Start-up chapters must receive recommendations for the funds from their Director-at-Large.
  2. Submitted applications must be complete and include a description of the proposed use of the funds and how this will benefit the growth and development of the start-up chapter.
  3. Start- up chapters may only receive the funding once but if they are unsuccessful after receiving the funds and then reform again they will become eligible again to re- apply for this funding as a new entity.

Selection Process

  1. Completed applications must be submitted to the SUNA National Office.
  2. Directors-at-Large will review the application and submit their recommendations to the SUNA Foundation to award the funds.
  3. The start-up chapters will be notified of their funds.

Recipient's Responsibility: Recipients will communicate progress on their chapter formation to their Directors-at-Large.

Applying for a Federal Tax Identification Number

  • Each chapter must apply for an Employer Identification Number (EIN), also known as a federal tax identification number (this is a 9 digit number).
  • To apply for an EIN, go to www.irs.gov.
  • Instructions for completing the form can be found at www.irs.gov/instructions (select SS-4 in html or pdf format).
  • There is no charge to apply for an EIN.

Maintaining A SUNA Chapter

  • In order to maintain active SUNA chapter status, a chapter must have six SUNA national members, submit a report on the activities in the chapter 60 days prior to each Board of Directors meeting, hold one meeting annually (this does not have to offer contact hours), and complete part 1 of the Chapter Review Form annually.

Developing Chapter Bylaws

  • All chapters must submit bylaws to the SUNA National Office at the time they are requesting chapter affiliation.
  • Chapters may adapt the current national bylaws or request a copy of another chapter’s bylaws to assist them in creating bylaws for their chapter.
  • Chapter bylaws must be reviewed / revised as necessary and submitted to the National Office where they will be kept on file.

Chapter of the Year Award

The Chapter of the Year Award acknowledges the chapter that has outstanding achievements in terms of:

  • Quality educational programs offered.
  • Growth in SUNA membership.
  • Involvement in community programs promoting awareness and education of urologic issues.
  • Full member active participation in chapter activities.
  • Innovative ideas to promote industry and member support.
  • Advancement of members to become certified by CBUNA.

Procedure

  • There is no limit to the number of times a chapter may win this award.
  • The winning chapter is selected by the SUNA Executive Committee through the information obtained from the completed Annual Chapter Review form submitted to the SUNA Secretary.
  • This form will be reviewed annually by the SUNA Secretary.
  • The Chapter Review Forms will be mailed with instructions and a cover letter to the Director-at-Large by the National Office.
  • The selected chapter is recognized with a special award during the Annual Conference.
